This is a really important moment for Karkat. He's taken over the group as leader, he's out exploring the game, and he happens to piss off this douche:

Jack Noir. Worth noting that Jack specifically went after Karkat because he 'can't keep his mouth shut,' so there has been some negative off-screen interaction already. Anyway, being Jack Noir, he stabs Karkat. Serious wound, bleeding everywhere.

Karkat freaks out, but not because he's hurt, but because he doesn't want anyone to see his blood. He's panicking and going on and on about how freakish it is, and reveals that because his red blood is a mutation, he's an outcast for it. We're not reading Karkat's specific dialogue here, but based on the sheer amount of exposition he's dropping and still has time to insist vague things are meant to taunt and punish him, he's having a major, loud panic attack all over the place about this.
Jack actually feels bad for him, and cuts his hand:

They shake hands and immediately become "in cahoots," which is the troll version of friends when you will probably stab each other in the back.
IMPACT: This was a really important moment for Karkat. He's been ashamed of his blood his entire life, and afraid of anyone finding out about it. He's really worried here not only of Jack's reaction but that his friends will find out and not respect him anymore.
But instead of everything going wrong, he meets for the first time in his life someone else who shares his blood color. He doesn't know yet that red is a normal color for carapaces (what Jack is), and also for humans, so it's not like Jack has shared his experience of being a mutant or anything, but it doesn't wind up mattering to him. The gesture alone meant a lot!
To say Jack is a shady character is a huge understatement, but this begins Karkat's intense hero worship. It will survive political intrigue and numerous "affectionate" stabbings. Jack pretty much straight up says he will betray Karkat and friends, that he hates him, etc., but Karkat is still really upset later on when an AU version of Jack kills him. Not that he knows any of that, but the fact that he latches onto someone like Jack Noir so much shows just how much this really meant to him.
On the other hand, Aather Karkat is coming off this memory with a different perspective. Before he knew about the hemospectrum, he bled plenty of times and didn't know there was a problem. He has about the same blood color as humans do, so he isn't in the position of never having met someone else who shares his color. Jack's gesture isn't quite as meaningful considered that. But just how upset Past Karkat is about his blood here, and how much he wants someone to relate to him, is pretty upsetting and hard to deal with, especially since the problem just seems stupid.
